Information on opting out of interest-based advertising

In addition to any opt-out feature provided by any of the services listed in this document, Users may follow the instructions provided by YourOnlineChoices (EU), the Network Advertising Initiative (US) and the Digital Advertising Alliance (US), DAAC (Canada), DDAI (Japan) or other similar initiatives. Such initiatives allow Users to select their tracking preferences for most of the advertising tools. The Owner thus recommends that Users make use of these resources in addition to the information provided in this document.

The Digital Advertising Alliance offers an application called AppChoices that helps Users to control interest-based advertising on mobile apps.

Users may also opt out of certain advertising features through applicable device settings, such as the device advertising settings for mobile phones or ads settings in general.

      Pine Cove, Inc. is required, by law, to maintain the privacy and confidentiality of your protected health information (PHI) and to provide our participants with notice of our legal duties and privacy procedures with respect to your PHI.

      Disclosure of your Health Care Information

      Occupational Injury
      If applicable, we may disclose your health information as necessary to comply with Appendix A Notice of Privacy Practices for Protected Health Information as covered in Pine Cove’s Safety In The Workplace Occupational Injury Summary Plan Description without your authorization, as long as it is for treatment, payment, or operations (TPO) as described therein.

      Emergencies
      We may disclose your health information to notify or assist in notifying a family member, or another person responsible for your care about your medical condition in the event of an emergency or of your death.

      Public Health
      As required by law, we may disclose your health information to public health authorities for purposes related to: preventing or controlling disease, injury or disability; reporting child abuse or neglect; reporting domestic violence; reporting to the Food and Drug Administration problems with products and reactions to medications; and reporting disease or infection exposure.

      Judicial and Administrative Proceedings
      We may disclose your health information in the course of any administrative or judicial proceeding.

      You have the right to request restrictions on certain uses and disclosures of your PHI. Please be advised, however, that Pine Cove, Inc. is not required to agree to the restriction that you requested.
      Upon request you have the right to have your PHI received or communicated through an alternative method or sent to an alternative location other than the usual method of communication or deemvery.
      You have the right to inspect and copy your PHI.
      You have a right to request that Pine Cove, Inc. amend your PHI. Please be advised, however, that Pine Cove, Inc. is not required to agree to amend said PHI. If your request to amend your PHI is denied, you will be provided with an explanation of denial reason(s) and information about how you can disagree with the denial.
      You have a right to receive an accounting of disclosures regarding your PHI made by Pine Cove, Inc.
      You have a right to a paper copy of this Notice of Privacy Practices at any time upon request.

      Changes to this Notice of Privacy Practices
      Pine Cove, Inc. reserves the right to amend this Notice of Privacy Practices at any time in the future, and will make the new provisions effective and available with all information that it maintains, and the new provisions will be posted on our website at www.pinecove.com. Until such amendment is made, Pine Cove, Inc. is required by law to comply with this Notice.

      Pine Cove, Inc. is required by law to maintain the privacy of your health information and to provide you with notice of its legal duties and privacy practices with respect to your health information.       Complaints about your Privacy Rights, or how Pine Cove, Inc. has handled your health information, should be directed to the Privacy Officer at (903) 561-0231.
