      Except as described in this Privacy Policy, Hearing Tracker only collects your personally identifiable information (“PII”) through this Website when you choose to provide such information, such as when you create an account by signing up, or when you fill in the details of your personal profile. Information you share about yourself in written comments or on forums may also constitute PII. PII can include your name, photograph, date of birth, zip code and e-mail address. Please be aware that elements of your personally-submitted PII, such as your name, photograph, and approximate location may be shared publicly and/or with your hearing provider. When submitting your PII in conjunction with a provider-initiated hearing aid review, it may be possible for Hearing Tracker users and/or your hearing provider to connect elements of your PII to the de-identified information submitted by your provider (such as the model of hearing aids you wear, your audiogram results, etc). Hearing Tracker will promptly remove any PII upon request. Hearing Tracker may use your PII to:

      - Address your requests for information, products or services;
      - Provide you with a list of nearby professionals; based on your location;
      - Administer your account;
      - Provide you with access to particular tools and services;
      - Mail or e-mail you information regarding products or services that we believe may be of interest to you; or
      - Respond to your inquiries and send you administrative communications.

      No Protected Health Information

      Hearing Tracker does not intend to collect any protected health information (“PHI”) as such term is defined by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Hearing aid dispensers initiating hearing aid reviews through the Website should never include PHI in those reviews. In the event that any PHI is discovered by Hearing Tracker on the Website, it will be promptly removed.

      Disclosures to Third Parties Assisting In Our Operations.

      Hearing Tracker may share your PII under confidentiality agreements with other companies that work with, or on behalf of, Hearing Tracker to provide products and services. These companies may use your PII to assist Hearing Tracker in its operations. These companies do not, however, have any independent right to share this information.

      Disclosures Under Special Circumstances.

      Automatically Collected Information and Anonymous Information

      Each time a visitor comes to the Website, Hearing Tracker collects some information to improve the overall quality of the visitor’s online experience.

      Aggregated Data.

      Hearing Tracker collects aggregate queries for internal reporting and also counts, tracks, and aggregates the visitor's activity into Hearing Tracker’s analysis of general traffic-flow at the Website. To these ends, Hearing Tracker may merge information about you into aggregated group data.
