Collecting information for various reasons is common for many websites, and it is vital (oftentimes required) for users to understand how their information will be used. With Forminator, you can create, customize, and implement a form where a user agrees to a privacy policy.

This brief article will show you how to easily set up a contact form that integrates with your legal documents in just a few clicks!

Create a Contact Form

We’ll start by creating a simple contact form. This is so we can also demonstrate how to include vital information about how the information is used. 

There are several ways to create a new contact form. You can start from Forminator’s Dashboard or the Forms section by clicking Create.

 

You’ll see your active forms, last submission, and more in the Forms area.

In this case, we’ll begin with a Contact Us form. We’re using this template to instantly set up most essential fields.

 

Want to design it starting with nothing? The Blank form might work best.

You’ll enter a name for the form, and then, as you can see, the rest of the form is already created for you. All the necessary fields, including First Name, Email Address, Phone Number, and Message are added.

 

Forminator’s drag-and-drop feature makes it a breeze to organize your fields however you’d like if you want to rearrange any of the fields.

Adding a Section About How Information is Used

Forminator allows you to add practically any fields you need. Whether it be for numbers, text, payments – you name it.

In this case, we need to add a section on what the information will be used for. And we want the form to have a checkbox that users must click to ensure they know what they consent to when submitting the form.

The easiest way to do this would be to include a Consent field. This will allow users to click on the links for more information on how the form is used. Plus, they will check a box indicating that they agree.

It starts by selecting the Consent field from the Fields options.

 

The Consent Field is ideal for telling users who opt-in what their information is used for.

Once implementing the Consent field, you can include a link to your website’s Privacy Policy and Terms & Conditions. Or, add customized text and other links. Whether with a link or text, indicate what their information is used for.

 

Add a description of what your users will consent to. In this example, there are links a user can visit. If using iubenda, you can simpley copy your privacy policy and terms and conditions link from within your dashboard and add them in here.

With the drag-and-drop feature, you can then add the field in any order you’d like on the form. We’ll put the Consent field at the bottom in this example.

 

You can edit the fields at any time by clicking the gear icon.

And check out what the form will look like by clicking on the Preview button. When previewing, you can see what the box that users must click to agree with the terms.

 

Change the word ‘Consent’ to something else if you’d like.

And that’s it!

That’s a quick walkthrough on setting up a basic contact form with consent. To go beyond just the basics, you can customize and tweak the form in numerous ways (e.g. add payments, require more information, etc.). Plus, edit any text, options, or fields – however you’d like.

Implementing the Form

Implementation of Forminator’s forms couldn’t be easier! When you’ve customized your form to your liking, simply click Publish.

 

Publish is located on the upper right.

Once you hit Publish, Forminator delivers a shortcode that you can use on any page or post.

 

Copy and paste the shortcode to any post or page you’d like!

Keep in mind that you can always go back and edit any form however you’d like. All published forms are quickly accessible from Forminator’s admin.

Implementing 3rd Party Apps to Collect Data

When organizing your contacts that opt-in on your form, you can include your favorite 3rd party apps. This includes MailChimp, HubSpot, Slack – you name it! After all, it’s up to you where you want your information to go.

It’s all in the Integrations area of Forminator’s dashboard under Applications.

 

There are 3rd party apps for automation, collecting emails, and much more.

Each app has its method of connecting, but all you have to do is click the Plus (+) sign to get started – and Forminator will give you a complete walkthrough on how it’s done.
